You will need to print this registration form, complete it and mail it with your payment. Payment by mail must be received no later than July 13, 2007. If payment is not received by July 13, 2007, you will need to register on-site.
You will not be registered for the conference until payment is received.
Cancellations and Substitutions
If you must cancel for any reason, notify our registration department in writing by fax (203) 286-1010 or e-mail registration@incisivemedia.com by June 19, 2007. Your registration will be refunded less a $100.00 processing fee. Cancellations after June 19, 2007 are non-refundable. You may transfer your registration to another person at any time by providing written authorization.
